556 Minimally Invasive Contracture Release Improves Function in Patients with Burn Contractures

Abstract Introduction Scar contracture bands after burns are a persistent problem that cause discomfort and physical aesthetic limitation. Standard treatment for in burn scars involves therapy, stretching, splinting laser treatment, as well surgical releases with local/distant flaps grafting. A minimally invasive transcutaneous approach the release of scar was initiated our institution 6 years ago. The method derived from techniques pioneered by Jose Daher Raul Gonzalez to interrupt platysma neck rejuvenation. With IRB approval we evaluated this new regarding long term outcomes. Methods retrospective review 45 patient charts conducted those who received subcutaneous May 1, 2016, through March 31, 2022. procedure introducing braided suture loop band moving it sawing motion until cuts multiple sites along contracture. Demographic data, total surface area (TBSA), location, amount increments, size, duration recorded. Concurrent treatments whether performed under anesthesia also noted. In follow up visits 3-12 months post patients were surveyed on satisfaction they noticed increased mobility procedure. Vancouver Scale administered pre- post-treatment. Patient range measured pre-and assess change at treated sites. Results Male female ratio 51%:49%, average age 38.8(6 68), 58% white . TBSA ranged 0.5% 85%, an 36.1 ± 22.3. applied 11 different spanning entire body, 39% involved 19.0 8.8 incremental releases, size 174.4 140.0 cm2. 93% procedures 76% done because laser, 84% site skin grafted. Only one reported bleeding post-discharge, none had infection puncture site. Out 20 responses, 13 being “satisfied” 3 “very satisfied”; “dissatisfied”. 19 noted increase did not. On average, 13.6 degrees. Conclusions described is versatile, safe, well-tolerated can help regain form function injury.
